*Provides home-based, clinical intervention to families reunifying after a foster care placement.
*
*Duties and Responsibilities*
* Provide in-home crisis oriented clinical treatment, education and support to at-risk families
* Provide a range of clinical, solution focused interventions to children, adults and families, using acceptable treatment methodologies consistent with the Family Preservation model
* Assume therapeutic responsibility for all cases on assigned caseload
* Actively participate as a member of team meetings; keep all members informed of case status and progress; provide oral and written reports as needed and make recommendations about case goals, service plans, action steps and activities for treatment plans
* Assist clients to clearly define presenting problems, devise methods and goals for resolution, assess their progress toward treatment goals and plan termination
* Present cases to supervisor for review of treatment plans and progress on a regularly scheduled basis
* Complete and maintain all reports, statistics and assignments as required
* Coordinate care by providing appropriate referral for clients requiring additional or alternative services and communicating with treatment team
*Job Qualifications*
*Education, Training, and Licensure/Certification*
* Master’s Degree in the Human Services field, Social Work preferred, with two years of family preservation program preferred; *OR* Bachelor degree in the Human Services field, Social Work preferred, and two years of family preservation program required
* Full or limited licensure with the State of Michigan preferred
*Additional Work Requirements*
* Travel required with ability to work in homes/community and utilize a flexible schedule to meet unique client needs.
Required to participate in an on call system of 24-hour availability to families
* Valid driver’s license with good driving record; reliable ongoing personal transportation suitable for safely transporting clients; current automobile registration and proof of automobile insurance
