Duties And Responsibilities:
Provides home-based, clinical intervention to families reunifying after a foster care placement.
Duties and Responsibilities
Provide in-home crisis oriented clinical treatment, education and support to at-risk families
Provide a range of clinical, solution focused interventions to children, adults and families, using acceptable treatment methodologies consistent with the Family Preservation model
Assume therapeutic responsibility for all cases on assigned caseload
Actively participate as a member of team meetings; keep all members informed of case status and progress; provide oral and written reports as needed and make recommendations about case goals, service plans, action steps and activities for treatment plans
Assist clients to clearly define presenting problems, devise methods and goals for resolution, assess their progress toward treatment goals and plan termination
Present cases to supervisor for review of treatment plans and progress on a regularly scheduled basis
Complete and maintain all reports, statistics and assignments as required
Coordinate care by providing appropriate referral for clients requiring additional or alternative services and communicating with treatment team
Qualifications:
Job Qualifications
Education, Training, and Licensure/Certification
Master’s Degree in the Human Services field, Social Work preferred, with two years of family preservation program preferred; OR Bachelor degree in the Human Services field, Social Work preferred, and two years of family preservation program required
Full or limited licensure with the State of Michigan preferred
Additional Work Requirements
Travel required with ability to work in homes/community and utilize a flexible schedule to meet unique client needs.
Required to participate in an on call system of 24-hour availability to families
Valid driver’s license with good driving record; reliable ongoing personal transportation suitable for safely transporting clients; current automobile registration and proof of automobile insurance
